WHEN: July 8 - 11, 2009

• 9am - 4pm (Wednesday to Friday)
• 8am - 12nn (Saturday)

WHERE: Kostka Extension

WHAT TO BRING:

• Any valid ID (with picture and signature)
• For PROXIES: Authorization Letter and valid ID (or photocopy of ID) of person being proxied
      * The Authorization Letter must indicate the following:
  • Date the letter was written
  • Date of validity of the letter
  • Specify the name of the proxy authorizing him/her to claim your yearbook for you on the date specified
  • Signature above the printed name of the person being proxied
